Get Enrolled

Once you have made the decision to work with Opportunity Services, please let your Case Manager or Vocational Rehabilitation Counselor know that OS is your choice.  Once that person knows you have selected OS, they will send our office the initial referral information that we need to begin the enrollment process.  Following a review the referral documents, we will schedule an intake meeting.  At this meeting we will define the expectations that you have for our services and develop goals and objectives which will be developed into your Individual Habilitation Plan or Employment Plan. 

Additionally, the intake meeting will serve to define your start date, potential pick-up time (if applicable), and answer any other questions that you may have.  So please bring all your questions and ideas to this meeting.
