Service Description:	Community Work Teams - Business work sites/ on-site supervisor Facility Vocational Community Living Program - 1:4 Staff ratio enhanced for community activities "Job of Your Own" Employed by business Job coaching Job retention Hourly service rate Seniors 1:2 Staff ratio Community activities Hourly rate Behavioral Support 1:2 Staff ratio enhanced when needed. Positive IABA Behavioral Program Rule 40 Programs only as absolutely required. Our focus is building positive skills.

Transportation:	Fixed route within 20 mile radius of Kasson site and throughout Dodge County.
